A statement of service (SOS) provides current and past employees with an accurate record of their employment with the department.
Current DECD employees can request a statement of service through the Employee Information Kiosk (EIK).
A statement of service generated through the EIK is accurate if you began employment with DECD after 14 July 2005. For any omissions or inaccuracies prior to this date please complete a statement of service (SOS) request form (PDF 187KB) and forward it to email@example.com.
Due to the large volume of requests please allow up to 6 weeks for your SOS to be prepared.
The SOS form will not provide information regarding long service leave (LSL) entitlements.
For LSL information call shared services on 8462 1333 and press 1 for payroll.
